General:
1. Application for purpose of Empanelment is dual process, both online as well as Physical. All documents required as per columns mentioned in online portal shall also be forwarded to O/o Director general AYUSH, Haryana sector-3, Panchkula.
2. The empanelment with Haryana Government is a voluntary process.
3. However, by submitting application along with written consent, the hospital agrees to abide by the terms and conditions of State Govt. Empanelment policies for AYUSH Hospitals and amendments thereof and is bound to follow the same in true letter & spirit.
4. Only legible and valid documents, duly signed and stamped by hospital authority.
5. Provision of any false information in application form or mismatch in availability of facilities/ manpower during physical surprise checks may lead to de-empanelment.
Filling of application:
1. Name & Address of the AYUSH Hospital shall be same as mentioned in NABH certificate.
2. Name and contact details of the Medical Director/ Medical Supdt. shall be provided for purpose of contact by authorities of State Govt. beneficiaries.
3. NABH certificate/ Entry level certificate along with scope of services shall be uploaded.
4. Level of accreditation and Scope of services shall be selected strictly as per NABH certificate/Entry Level uploaded.
5. Details of all the doctors working in the hospital shall be provided, especially corresponding to the specialties for which empanelment is sought, by using option of addition.
6. In case, the lab services are outsourced under MoU/agreement, MOU must be provided.
8. Bio-medical Waste Authorization Certificate and Consent to operate under the Water Act & the Air Act, issued by Pollution Control Board, shall be provided.
9. NOC for Fire safety, issued by Govt. Authority, shall be provided.
10. Copy of PAN card of the hospital/ proprietor shall be provided.
11. Building Plan approval/ Occupancy Certificate, issued by Govt. Authority, shall be provided.
12. Lift safety certificate, issued by Govt. Authority, shall be provided.
13. Ambulance services, in-house or outsourced, total number of ambulances and registration certificate, MOU for each, shall be provided.
14. Canteen/ catering services, if available, shall be registered under the Food Safety Standards Act, 2006 and a copy of the same shall be provided.
15. Non refundable application fee of Rs 5000/ shall also be uploaded.
- The status of application may be tracked using the registered mobile number.
- Successful submission of application does not guarantee empanelment. The application will be processed by various authorities, and only after approval of competent authority, written orders will be issued from the O/o DG Ayush, Haryana.
